We are the provider of personal care and Care Management to older adults who live independently in their own homes. Services are provided to ensure the clients can live independently, remain safe, navigate health events successfully, and maintain optimum health. The clients are self-pay or have long-term care insurance.
This position provides an opportunity to:
· Build relationships with the clients.
· Provide clinical oversight to Professional Caregivers.
· Expand the implementation of a holistic model of care.
· Provide advocacy and care management.

Full time 4 days 8:30 am - 5:00 pm
Light on-call rotation with two other nurses
Pay is $32 - $34 per hour depending on experience, billed RN Care Management paid at $50, and on-call pay.

Must have at least 5 years of nursing experience with at least 2 years of geriatric care management experience.
Previous supervisory experience required.
