SUMMARY
Under the direction of the Program Manger this position is responsible for the work related to Compass Affordable Housing Community Housing Programs. This position is responsible for building community and funder partnerships, grant writing and includes attending meetings, implementing community events, arranging meetings and trainings, case management and coordinating other activities pursuant to grant or other CAH requirements. This position also ensures protection of the organization’s financial assets and compliance with CAH directives and applicable grantor, federal and state requirements.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Assist Program Manager in grant and grant reports to support CAH Programs
2. Assist Program Manager train, oversee and assign work to Interns
3. Facilitate the maintenance and implementation of Homeless Management Information System and including maintaining the ART HMIS reporting license
4. Work with the Pathways Home database, Call Point (Coordinated Assessment Initiative) SPDAT assessment tool to provide comprehensive services to client/tenants
5. Complete HMIS annual report and work with Continuum of Care committees for Housing Inventory and Point-in-Time counts.
6. Identify existing and propose new data tracking systems needed to provide summary data for program reports related to CAH systems.
7. Providing high-quality data analysis that informs, supports and improves the delivery of services.
8. Develop working relationships with various systems of care
9. Provide the direct services of outreach and engagement
10. Maintain strict adherence to the confidentiality of each and every client
11. Maintain records and files related to tenant services and CAH programs
12. As directed, maintain regular relationships with clients/tenants
13. Work with walk-ins to assist in accessing needed services
14. Provide Case Management duties as follows:
· Facilitate and train staff and interns on Individual Service Plan Meetings with clients, including administration of the SPDAT
· Maintain thorough case notes on all client interactions and advocacy work
· Maintain current HMIS systems and reports on clients
· Assist clients with building skills that are relevant to their goals (i.e. assisting with job search, resource applications, and other navigation of resources)
· Attend re-engagement meetings with Community Housing Specialist in times of potential client conflict or crisis
· Facilitate quarterly meet ups for clients
· Oversee SOS, Gospel Rescue Mission, Homeless Court and other referral programs currently utilized by clients.
15. Provide support to CAH staff, if applicable
16. Ensure effective and professional communications and relationships with CEO, stakeholders, staff, interns and project team members
17. Track and report weekly on work completed and timeline
18. Follow personnel, financial, and confidentiality policies
19. Additional duties as necessary to assist with the operation and management of the CAH programs
QUALIFICATIONS
· Familiarity with human and/or social service programs (e.g. homeless, aging, mental health, adult protective services, child protective services, etc.).
· Bachelor's degree in social work (obtained or in school), or a related field or 2 years’ experience in client services preferred.
· Team player (flexible, open-minded, willing, committed)
· Commitment to team building, learning best practices, and continuing education
· Ability to communicate effectively with and support existing and potential clients.
· Professional communication skills and ability to build and maintain collaborative relationships with a diverse set of agencies and organizations.
· Ability to multitask and work on more than one project and complete assignments
· Valid driver's license and ability to life 50 pounds
· Strong oral, written, and computer skills
· Strong collaboration and facilitation skills
· Enthusiasm in working with people
