Requirements:
• Current driver’s license, auto insurance & clean driving record
• Ability to pass a background check
• Willingness to assist individuals with personal hygiene/toileting/feeding
• Excellent interpersonal, verbal, and written communication skills
• Technology competency
• Ability to bend, squat, kneel, lift, twist, push, and pull to complete assigned tasks
• Ability to walk and stand for long periods
• Ability to drive for individual transportation to and from home or on community outings
*This Specific position assumes the following schedule: Monday - Friday from 8:45am - 4:30pm
ESSENTIAL FUNCTIONS
1. Provide dignified direct care and service in accordance with each person’s individual plan (IP) and in compliance with UCP policy, procedure and code of professional ethics. This includes activities such as eating, toileting, transferring to/from adaptive equipment, transporting in a wheelchair, hand over hand assistance, accessing the community and participating in activities within the program and the community.
2. Ensure the health and safety of each individual supported by responding to and acting on any accident, emergency, incident, and medical problems immediately and in accordance with UCP policy, procedure and code of professional ethics, ensuring necessary follow-up communication and confidentiality.
3. Monitor and maintain specific dietary requirements in accordance with individuals’ individual program guidelines.
4. Provide skill-building and sensory stimulating activities that are appropriate and centered on enhancing and enriching each participant's life.
5. Develop a rapport with Individuals and cultivate a safe and supportive relationship.
6. Be an advocate and a role model for the individuals and encourage attitudes and behaviors that enhance community inclusion.
7. Provide transportation to/from the program, the individuals homes and the community, using your vehicle or an agency vehicle.
8. Accurately and thoroughly complete your documentation on each shift by recording each individual’s progress and documenting other required information in Therap and other electronic recordkeeping sources.
9. Maintain a clean work environment, including vehicles and program facilities.
WORK SETTING AND CONDITIONS
This position is primarily based in a Day Program setting. Will require occasional work in the community with the individuals. Responsibilities occasionally may require an adjusted work schedule in order to meet the program needs.
