Lazarus Street Medicine Outreach Case Manager (CM) An Exciting Opportunity to Make a Difference!

Camillus Health Concern is seeking a compassionate, dedicated, and mission-driven Case Manager to join our Lazarus Street Medicine Outreach Project.

This is a unique opportunity to take your case management skills beyond the traditional setting and make a meaningful impact in the lives of individuals experiencing homelessness and other vulnerable populations throughout our community.

As part of the Lazarus Project, you will work directly in the community alongside a multidisciplinary team to connect individuals with healthcare, behavioral health services, housing resources, social services, and other essential support.

Position Summary

The CM provides day-to-day street-based outreach case management services through psychosocial assessment, intervention and follow-up for Camillus Health Concern's (CHC) Lazarus Project, a community-based initiative designed to bring health care, outreach, and supportive services directly to individuals experiencing homelessness and other vulnerable populations who face significant barriers to accessing traditional health care. The CM Is responsible for developing relationships with individuals experiencing homelessness, and connecting individuals to appropriate medical, behavioral health, dental, pharmacy, social-service, housing, and other resources. The position requires an individual who is comfortable working outside of a traditional case management setting and who understands that successful street medicine requires consistency, trust, compassion, persistence, and strong relationships. The CM works closely with the members of the Lazarus Project team, clinical providers, nursing, behavioral health, and social services to ensure that individuals engaged through the Lazarus Project have a pathway from street outreach to ongoing care and supportive services.

Responsibilities
  • Meet individuals where they are and provide a welcoming pathway into health care and supportive services and develop relationships with individuals experiencing homelessness through consistent and respectful engagement and maintain professional boundaries.
  • Provide patients with supportive short-term and crisis counseling.
  • Conduct full assessment of each patient’s medical, financial, social and other needs.
  • Provide Photo Identification cards.
  • Maintain a visible and consistent presence within the community.
  • Promote trust and continuity by ensuring that outreach commitments are followed through.
  • Provide information and education on the Social Security Disability and Medicaid processes and other government programs.
  • Provide comprehensive case management services to clients through intervention, referrals and follow-up including patients who have been hospitalized.
  • Assist patients in obtaining medically necessary durable medical equipment and supplemental items including wheelchairs, hearing aids, eyeglasses, orthotics/insoles, and other approved medical supplies, as needed.
  • Provide care planning through identifying, implementing and management of community service programs for client including Camillus House services.
  • Monitor client compliance with care plan and medication regimens, as well as ensuring that service providers are rendering services as requested.
  • Follow-up with clients who have relocated to less supportive housing situations.
  • Enter and track data in the HMIS system accurately, consistently, and in a timely manner. Good data quality is important for client care, service coordination, reporting, and tracking outcomes.
Requirements
  • Bachelor’s degree in social work or related field; previous experience and Certified Behavioral Health Case Manager (CBHCM) certification will be considered.
  • Knowledge of and experience with public health issues.
  • Bilingual (English/Spanish) preferred, Creole a plus.
  • Computer literate; previous experience with Electronic Medical Records (EPIC).
  • Community and public health experience.
  • Culturally sensitive to the social, psychological, and healthcare needs of underserved populations.
Preferred Qualifications
  • Commitment to the Mission and Values of the Hospitaller Order of St. John of God.
  • Experience coordinating community programs or multidisciplinary teams preferred.
  • Experience in an FQHC, community health center, street medicine program, homeless health program, or other safety-net organization preferred.
  • Experience developing community partnerships preferred.
  • Knowledge of Miami-Dade County community resources and HMIS (Homeless Management Information System - countywide information system used to document and coordinate services for people experiencing homelessness or who are at risk of homelessness).
Hours & Schedule
  • Full-time position with a primarily weekday schedule.
  • Must be available for approximately 2–3 hours on Saturday and Sunday mornings to support Lazarus Project outreach activities.
  • Weekend hours will be accommodated through a flex schedule, with corresponding time off during the regular workweek.
  • Flexibility is required based on program and community outreach needs.
