We are currently seeking a dedicated Home Care Clinical Coordinator to join our growing team.

Position: Home Care Clinical Coordinator
Location:
1029 Teaneck Road, Suite 3D
Teaneck, NJ 07666
Type: Full-Time

Key Responsibilities:

  • Maintain and organize client files

  • Monitor file compliance and ensure all required documentation is current

  • Track client assessments, reassessments, care plans, and supervisory visits

  • Coordinate with staff to obtain missing documentation and required updates

  • Assist with state surveys, audits, and compliance reviews

  • Review records for completeness and accuracy

  • Maintain electronic and paper records in accordance with company policies

  • Support the office team with administrative and compliance-related tasks

  • Communicate with clients, caregivers, and healthcare professionals regarding required documentation



What We're Looking For:

Experience:

  • 2+ years of home care, healthcare administration, compliance, or related experience preferred

Education:

  • High school diploma required

  • Healthcare administration or clinical background preferred



Skills:

  • Strong organizational and communication skills

  • Excellent attention to detail

  • Ability to manage multiple deadlines and priorities

  • Computer proficiency and experience with electronic record systems
