Managed a case-load of 65-75 elderly and disabled clients for this PACE organization. Main objective was to assure clients were living in an appropriate level of care. Met this objective by conducting home visits with clients and maintaining contact with family and caregivers.
Highlights of Contributions:
Completed initial psychosocial assessments, MMSE’s, and GDS’s upon intake, every 6 months, and needed.
Developed care plans for clients.

Maintained a case-load of 85-90 clients who were enrolled in
the HCBS, HCA, or the Brain Injury Waiver through the state of Colorado. Main objective was to assist these clients to receive appropriate services through these programs.

Completed 6 month home visits in clients’ homes to verify services and to review plans of care.
Assisted clients with ordering DME and scheduling home modifications to make their homes more accessible.
Conducted quarterly calls to clients to verify services and to maintain contact with clients and their caregivers.

Scheduled and attended care conferences for residents.
Assisted residents with discharging to less restrictive environments that could provide services for their mental health needs.
